During trainings (Monday & Thursday)
Wed, 8 Dec from 4-6pm at Tennis Club House (Canteen 2 block, level 3 facing the overhead bridge to tennis court)

If you are unable to collect your forms from me on those days, please contact me
Those who have signed their forms can pass to me your forms on those days too.
